Return of questionnaire procedure No: 012D
1. Introduction
This SOP offers guidance on the procedure to follow after the
return of the GP despatched questionnaire.
2. Responsibilities
Research nurses are responsible for ensuring that the procedure
following the return of a questionnaire is correctly followed.

Ensure to
follow the steps laid down by figure 1, following the return
of a questionnaire.

Following the return of a questionnaire with a family history
suggesting an affected sibling

= Telephone Index case
Complete
family screening questionnaire and ask index case to call sibling
to confirm interest. Indicate you will call to verify interest
in a day or so.
